I'm Dr.Lindsay oglE, Metabolic Health expert

As a board-certified family medicine and obesity medicine physician, I am committed to helping adults in Missouri who are facing challenges related to metabolic conditions, including obesity, PCOS, prediabetes, diabetes, and metabolic syndrome. In this post, I will explore the role of bariatric and metabolic surgery in managing obesity and metabolic disorders, especially in light of the availability of new GLP-1 medications.

The Role of Bariatric and Metabolic Surgery

With advancements in obesity treatments, including medications like GLP-1 agonists (e.g., Wegovy and Zepbound), many wonder about the necessity of bariatric surgery. However, I believe that surgery remains a vital option for many patients. Here’s why:

1. Surgery Is Not "Cheating": A common misconception is that surgery is a shortcut. In reality, obesity is a chronic medical condition requiring diverse treatment strategies. Surgical options are just one of many effective solutions available, alongside lifestyle changes and medications.

2. Surgery Is Not A Last Resort: Many patients benefit from considering surgery earlier in their treatment journey. For some, it could be an excellent option during the initial consultation rather than waiting until all other methods have been exhausted.

3. Safety of Surgery: Today's bariatric surgeons are highly skilled, often performing these procedures exclusively. With improved techniques and adherence to safety standards, the peri-surgical risk associated with bariatric surgery is comparable to that of routine surgeries like gallbladder removals.

Who Qualifies for Bariatric Surgery?

Bariatric and metabolic surgery is not a one-size-fits-all solution; qualification is based on various factors. In general:

- Body Mass Index (BMI) of 40 or greater
- BMI of 35 with type 2 diabetes or having unsuccessfully tried other weight management interventions also makes you eligible.

It's essential to have one-on-one consultations with qualified surgeons, as they will tailor advice to individual needs, including discussing insurance coverage, which may not always guarantee the procedure.

Types of Bariatric Surgery

The two most common procedures in the U.S. are the sleeve gastrectomy and the Roux-en-Y gastric bypass:

- Sleeve Gastrectomy: Involves removing a portion of the stomach but preserves the intestines. Patients can expect around a 60-70% excess body weight loss. While the procedure has a lower risk of malabsorption, it may carry a higher risk of acid reflux due to the physical changes in the stomach.

- Roux-en-Y Gastric Bypass: This method creates a small pouch from the stomach and bypasses a significant portion of the intestines, leading to reduced calorie absorption and often greater weight loss. It's particularly beneficial for patients with type 2 diabetes, resulting in high rates of disease remission.

Cost and Long-Term Considerations

While the cost of surgery can differ, it’s crucial to view it within the context of long-term health benefits. Medications like GLP-1 can cost between $350 to $1,000 monthly indefinitely. Surgery has a higher initial cost, but may present a more cost-effective solution in managing obesity, reducing the burden of chronic conditions and enhancing overall quality of life.

Conclusion

At Missouri Metabolic Health, we believe in an integrated approach to treating obesity and metabolic disorders. Bariatric and metabolic surgery may play a critical role in your journey toward improved health, often complementing medication strategies for the best outcomes.
